- Associate’s degree in Social Work or other related discipline
- Associate Mental Health Professional/ Paraprofessional (Meets definition with education and experience)
- At least two years of prior experience working in the mental health, substance use or behavioral health field with youth and families

At Selfless Foundation, Inc., we are proud to offer Elevate, our Individual & Transitional Support Service—designed to help individuals 16 & older live independently in their homes and communities.
As an Individual and Transitional Support Specialist, you will provide up to 15 hours per week of face-to-face, critical non-medical behavioral health support tailored to each person’s needs. You will assist individuals enrolled in the program by connecting them to employment, housing and other independent living resources to sustain their independent living.
Our Individual and Transitional Support services are designed to empower individuals to thrive with the support they need, right in their communities.

ResponsibilitiesItem Link
- · Contributes to Selfless Foundation, Inc. mission, vision, and program philosophies by way of work product and professional behavior with both internal and external consumers .
- · Provides a minimum of 15 hours per week of face to face services with consumers in the program to improve independent living skills, job skills and employability.
- · Assist consumers with connecting to resources to address any deficits in their life.
- · Aid and assist consumers with enrolling in higher education, certification programs and or job training programs.
- · Meets face-to-face with consumers in their own home, school and community for the provision of all services
- · Reassesses the needs of the consumers every 90 days and family with the team and modifies the service plan to reflect the progress or lack of progress.
- · Keeps the family and team members informed on progress toward achieving Service Plan goals.
- · Facilitates consumer efforts to obtain any financial assistance or services for which they may be eligible under federal, state, and local regulations.
- · Maintains accurate and concise documentation of contacts with consumer (Documentation is to be done within 48 hours after services has been delivered.)
- · Maintains an empathic, yet professional, relationship with consumers and families and strives to keep families engaged in all services.
- · Maintains all medical record documentation in the electronic record and must make sure that they are adhering to requirement set by the state of North Carolina Records Management code.
- · Adheres to all current federal & state laws, as well as Selfless Foundation, Inc. Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA)
- Reports immediately any suspected incidents of child or elder abuse or neglect to the Child Abuse Hotline and local authorities.
- · Establishes relationships and communicates with consumers, caseworkers/probation officers, family members, and staff.
- · Complies with all initial and annual training requirements
- · Attends staff / agency meetings as required.
- · Attend one hour of individual supervision weekly.
- · Attend two hours of group supervision monthly.
- · Completes other duties as directed by the Program Manager.
- · Enters demographic data, contacts made, and other required data for all newly assigned young adults in a timely manner i.e., 48 hours.
- · Works with consumer age 16 and older to develop a Crisis Plan to ensure the safety of the consumer and community.
- · Works approved skills curriculum with consumers to increase independent living skills, job readiness and vocational training.
